Forms For First Year Graduate Students

EEC 290C Form
(3 Professor Meetings) If you do not have a major professor, you are required to meet with three professors to discuss their research and get their signatures by the first Monday in December.

Remedial Course Form
For students without a B.S. in EE/CE

Forms for PhD degree

Application for Graduate Preliminary Evaluation for Admission to the PhD program
Due: Middle of the first Fall quarter you are admitted

Major Professor Selection Form
Due: By the end of the first quarter of enrollment. *Note* Even if the major professor is assigned before admission, you still need to submit this form.

Program of Study Form
Due: Within the first 7 days of the Fall quarter immediately following the passing of the Doctoral Preliminary Examination

Doctoral Guidance Committee Form
Due: After passing the PhD Preliminary Evaluation

Qualifying Exam Application Form
Due: When you have completed all of the necessary course work and prepare to do the QE exam

Ph.D. Advancement to Candidacy-Plan B
Fee payable at the Cashier’s Office in Dutton Hall.
Due: Within one month after successful completion of the qualifying examination

ECE Filing Fee Status Form and Graduate Studies Filing Fee Application & Regulations
Due: Two quarters before exit seminar

Exit Seminar Verification Form
Due: After the completion of the exit seminar

Ph.D. Checklist
Be prepared to spend 15-20 minutes in the Office Graduate Studies office while the thesis and degree requirements are checked. It is REQUIRED that you call and make an appointment with your Student Affairs Coordinator to file the dissertation. Click here to contact your Student Affairs Coordinator. Make sure you’ve completed this entire checklist before your appointment.Please remember that you may NOT submit your dissertation until you’ve completed your Exit Seminar.

Forms for MS degree Plan I (Thesis)

Major Professor Selection Form
Due: No later than start of the third quarter of enrollment

M.S. Plan-I Program of Study Form
Due: By the third quarter of enrollment

Thesis Committee Form
Due: Before completion of the thesis work

M.S. Advancement to Candidacy Form (Thesis)
Fee payable at the Cashier’s Office in Dutton Hall.
Due: During the quarter when all the non-thesis requirements will be completed

Due: No later than start of the third quarter of enrollment

M.S. Plan-II Program of Study Form
Due: By the third quarter of enrollment

M.S. Plan-II Advancement to Candidacy Form
Due: During the quarter when all the coursework requirements will be completed

Application Form for Comprehensive Exam Requirement for M.S. Plan-II
Due: After advancing to candidacy

Qualifying Exam for Comprehensive Exam Agreement Form
Due: If you are a Ph.D. student and want to request permission from your PhD Qualifying Exam Committee to combine the comprehensive exam for the MS Plan II degree with the PhD qualifying exam.

Verification of Completion of Comprehensive Examination Requirement for M.S. Plan-II
Due: After passing the comprehensive examination
